Failed emissions test

my '04 1.6TDCi has just failed it's NCT (Irish equivalent of the MOT) on emissions, namely smoke at 5.04/m against a limit of 3.00/m (as an aside how can smoke be measured in /m ???).

Any idea what it could be? There have been no other problems with the car at all, including loss of power or limp-home mode or anything nasty like that. It doesn't get used too much, only at weekends really if that's relevant.

One quote I have heard many a time and I stick with it, if you own a diesel and you have a mot due, then just before handing over the keys, give the car a good blast.
By that I mean you should go for a run in the car and for example instead of driving at 30 mph in 4th or 5th gear, do it in second or third gear. High revs and a good run will bring down the emissions.
your so right lawe give the car a good run. i had this problem with my other car and was told by the mechanic to put some NO SMOKE in the fuel tank then give it a run just to get it all mixed up thenit will pass mot. and it did pass the garage said itwas one of the cleanest test they had done as the car has 170k on now still going strong.
